  • I want to take part in this event and sign this document in consideration of being given the opportunity to engage in these activities.

    I understand that:
    • The activity may be physically and mentally intense.
    • The activity can be dangerous if I do not follow guides’s directions.
    • The possibility of injury to myself and others exists. As an adult I accept this risk.
    • Some of my guides may be trainees gaining experience to fulfill their qualifications. Trainee guides will at all times act under the supervision of a fully qualified quide.
    • I am responsible for the security of my personal belongings at all times, even if left in storage.
    • I am under no obligation to take part in any or all of the activities.
    I confirm and agree that:
    • I am fully aware of the risks to myself and others involved in this event/activity. I can talk to the guides regarding any concerns and about what’s involved before I start.
    • I will bring all the required equipment & clothing, I understand that boots are essential. If I have any issues I will contact in advance.
    • I am physically fit and have no injuries which may affect my safety during the activities. (I understand that if I have ANY concerns about my fitness I can and will inform the Event Manager)
    • I will comply with the instructor’s guidance at all times.
    • I will inform in writing or email within 48 hours of any injury sustained on these activities.
    • I will not be under the influence of alcohol during the activity.
    • I understand that guiding is provided by qualified independent guides who carry their own insurance.

    2. Do you frequently have pains in your heart and chest?
    3. Do you feel faint or have spells of severe dizziness?
    4. Has your doctor ever said your blood pressure is too high?
    5. Has your Doctor ever told you have a bone or joint problem, such as arthritis, that has been aggravated by exercise or might be made worse be exercise?
    6. Is there a good physical reason not mentioned above here why you should not follow an activity program even if you wanted to?
    7. Are you over the age of 65 and not accustomed to vigorous exercise?

  • I hereby release, remise and forever discharge from any claims and liabilities whatsoever without limitation that I might have against Go Wild ltd t/a, the owners of the property on which activities take place, staff, independent guides, subcontractors and I make this release on behalf of myself, my heirs, executors, assigns and administrators.
